SCENARIO

Read the following scenario:

You are a nurse leader on a unit with an interprofessional case management team working with congestive heart failure patients. The team was established one year ago, and everyone has worked well together. However, three months ago, three members left the team due to changes in their roles. Over the last two months, three new members have joined the team to replace the members who left. Since the new members joined the team, there has been tension and disagreements among the members. Sam, one of the team’s original members, is discouraged and wants to leave the team.

INSTRUCTIONS

Consider how you, as the nurse leader and manager, would advise Sam.

Write a 2-page plan on how you would assist Sam with the tension and disagreements he is experiencing with other team members. Your plan should include the following:

  • Competencies focused on patient-centered care
  • Teamwork and collaboration
  • Safety.

Include a summary of the team’s development stage and strategies to help the team progress.
